Sacramental Certificate Request

To request your certificate, you must include a copy of a valid, signed photo ID.

Requests will not be processed without a valid ID.

Please allow 7-10 days for processing.

The certificate can be picked up in person, mailed to you, or mail directly to the church who is requesting your sacramental record.

Please note that if you are over the age of 18, you must request your own certificate and provide a copy of a valid, signed photo ID. Parents are allowed to request certificates for a minor up until the age of 18.

Certificate can not be emailed or faxed: the raised seal that authenticates the document does not transmit electronically.
